Hailey works at the placement office responsible for long-term care facilities. When beds are made available in the community, Hailey needs to know about them immediately to ensure a client from the waitlist is placed into the bed as efficiently as possible to maximize occupancy rates. The process involves multiple steps with communication between the health jurisdiction, the client and family, and the long-term care home. In addition, Hailey regularly gets calls from hospital clinicians and her executive team with questions about the available beds.

When a long-term care home discharges a patient from their facility, they can quickly create a bed in the system and specify the supports provided. A new discharge notifies Hailey that a bed is available so she can match the next appropriate client from the waitlist. The LTC home can now view the client that Hailey has matched and can ensure they can provide care (with automated communication and status updates between the two). A declined match informs Hailey to find the next client. If it is accepted, she can offer the bed to the client’s family. Each step of the process receives tracking and reports. Once an appropriate client receives approval from the facility and the client has accepted, bed arrangements begin. Hailey can then report precisely how long it took to fill that bed.
