Wound Care Clinicians in Fraser Health Authority to Use Strata PathWays for Referrals

Wound Care Clinicians to utilize benefits of Strata PathWays software to improve patient referral process.

Strata Health is pleased to announce that Wound Care Clinicians (WCC’s) in the Fraser Health Authority have opted to utilize Strata PathWays software to improve existing challenges in the referral process between acute and long-term facilities. Wound Care Clinicians (WCC’s) are healthcare professionals who have received specialized training to treat and care for every kind of wound. They are experts at understanding the skin’s natural healing capabilities and work to help patients accelerate the healing process. However, WCC’s within the Fraser Health Authority have been encountering several challenges with the existing referral system, including:

• Security firewalls blocking access to sending files, resulting in reliance on faxing referrals, which was often unreliable or difficult to read.
• Believing documentation had been sent, only to discover the firewall had blocked it. In some instances, care providers were not aware that a message hadn’t been received.

These issues were understandably creating inefficiencies and difficulties for care providers in ensuring timely and accurate care. Strata PathWays promises to offer a solution for these challenges, allowing WCC’s in the Fraser Health Region to focus their time and attention on patients, and not on solving IT issues. Through this initiative, Strata PathWays will be made available to 170 users at 80 organizations, providing a reliable web-based solution for WCC’s to securely share referrals, create a treatment plan and track follow-up care.
