Customer Spotlight: Southlake Selects Strata Health to Streamline Hospital-to-Home Transitions

We are proud to announce that Southlake has chosen Strata Health to optimize patient referrals and transitions from Southlake Regional Health Centre into the Southlake@home program.

Southlake serves a vast population of nearly 1.5 million residents in York Region and Simcoe County, Ontario, Canada, and stands as a pillar in the healthcare ecosystem for these communities. As demands on acute and long-term care services continue to escalate, the Southlake@home program empowers patients to receive ongoing care from the comfort of their homes after their hospital discharge.

Strata Health’s platform will be used to initiate and manage homecare referrals by a diverse team of professionals at Southlake. Notably, leading homecare providers like Luma Care, CHATS, LOFT, CBI Health, and SE Health will utilize Strata Health to efficiently handle incoming referrals.

The Strata Health platform is poised to enhance care coordination, streamline operational processes, and improve the overall patient journey, ensuring that patients who are ready for discharge can safely transition back home with the necessary care, while also relieving acute pressures and freeing up hospital beds for individuals in critical need.
