12 August 2025 – Strata Health is proud to announce the successful go-live of a Home First assessment workflow at Burnley General Teaching Hospital and Royal Blackburn Teaching Hospital, part of East Lancashire Hospitals NHS Trust (ELHT).
The digital workflow, built on the Strata PathWays platform, transforms how patient assessments are conducted and shared. Care teams can now complete Home First assessments in the patient’s home, with results securely shared in real time with the community service providers supporting each patient’s recovery journey.
This digital approach also empowers ELHT’s Integrated Care Assessment Team (ICAT) to:
- Streamline communication between hospital and community providers
- Improve outcome reporting with faster, more accurate data capture
- Reduce administrative burden by eliminating manual reporting processes
