March 26, 2013 – London, United Kingdom – It was announced nationally today The Rotherham NHS Foundation Trust (TRFT) and Strata Health UK Ltd. have been shortlisted for this year’s respected Care Integration Awards. The Awards have received their highest number of entries in history. The competition was fierce; and this nomination showcases the hard work and great strides taking place in the trust.

With the establishment of seamless care pathways representing such a significant area of focus in the health service, the Care Integration awards help to highlight the innovation and courage with which this challenge is being addressed across the UK. The best pathways will achieve the key recommendation in the Francis Report; that NHS oranisations make the provision of patient-centred care their priority.

The awards recognise those organisations and individuals who are at the forefront of offering integrated care to ensure that local health and wellbeing needs are better understood and addressed.

In Rotherham, the musculoskeletal care pathway is one of several key areas TRFT is focused on in bridging care transition planning across multidisciplinary teams (MDT’s) within the hospital and community settings.

Benjamin Chico, Programme Manager for The Rotherham Foundation Trust commented, “Strata has underpinned efficient e-referrals between the Trust acute and community services including local social services. It has supported earlier discharge through it’s e-referral system, both through the system and also via a dashboard directory of services made available to the hospital wards. Our nursing staff love it!”

One of the areas of significant gain has been in mobilising MSK patients to the right interventions, at the right time, across multidisciplinary teams spanning the hospital, community and social care. Local referral routes across health and social care MDT’s were process mapped to derive the evolving local best-practice across this emerging health and social care ecosystem.

Clint Schick, Managing Director Strata Health UK added, “the larger strategy behind establishing paperless joint-planning and communication (across all stakeholders; underpinned by technology) was to enhance patient reablement within the community, and regain/maintain mobility, independence and quality-of-life in the safest environment possible”.

These lofty outcomes were supported by the (Strata Health) system, which enabled clinicians to communicate easily and consistently with one another to deliver a transparent service; jointly as a unified team.

High-risk groups (particularly older peoples) were at the heart of the teams’ focus. The capture of metrics to rapidly mobilize patients along the necessary care pathways, and then transition to required services within appropriate providers has achieved better outcomes, consistently. Early assessment/diagnosis and timely intervention have also been critical to success. The dynamic process of changing service responsibilities and locations throughout clinical pathways make the project an ongoing and dynamic piece of work; one that will continue to evolve with the needs of patients in Rotherham.
