New Clinical Nursing Pathway Introduced in Fraser Health Authority Care Homes

Fraser Health Authority to Utilize Strata PathWays to Improve Acute Clinical Nursing Care Referrals in Long-Term Care Homes. 

(August 2021) Strata Health is pleased to partner with Fraser Health Authority Care Homes in implementing Strata PathWays technology in acute care service requests for patients in Long-Term Care. Strata PathWays will now be used by Clinical Nurse Specialists (CNSs) to receive acute care referrals for long-term care patients and assess the best course of action. Prior to introducing Strata Health technology, the clinical nursing pathway referral process typically relied on a mixture of faxes and phone calls. The steps were cumbersome and often required duplicating work to process simple changes to a patient’s file. With Strata PathWays, the entire clinical nursing pathway referral process can now be electronically contained. Updates can be sent automatically without needing to repeat work, and Clinical Nurse Specialists can be supported in providing timely and accurate referrals. As a reliable web-based solution, Strata PathWays provides the ability for CNSs to share referrals, patient treatment plans and track follow-up securely. Through this initiative, Strata Pathways will now be available to 175 Clinical and Care home users across the Fraser Health Authority.
