National Case Management Week 2026
As we celebrate National Case Management Week 2026, I am honored to recognize and thank the incredible case managers whose expertise, compassion, and commitment make a difference in the lives of patients and families every day.
As a former case manager and now a healthcare technology leader, I have had the privilege of witnessing the impact of this profession from multiple perspectives. While my role has changed, my appreciation and passion for case management has only grown. I know firsthand that behind every successful transition, every coordinated plan of care, and every improved patient experience is a case manager helping to drive meaningful outcomes.
This year’s themes from the American Case Management Association, “Case Management: Driving Outcomes,” and the Case Management Society of America, “Reconnect. Recognize. Reignite. Rise.” perfectly capture both the value and the future of our profession.
Reconnect
Case management has always been about connecting people to the care, resources, and support they need to achieve their best possible outcomes.
This week offers an opportunity to reconnect with the purpose that brought us to this work: advocating for patients, supporting families, navigating complexity, and ensuring that every individual receives the right care at the right time and in the right setting.
In an increasingly complex healthcare environment, case managers remain the vital connection point across providers, care settings, and communities. Every day, you help create clarity where there is uncertainty and coordination where there is fragmentation.
Recognize
National Case Management Week is also a time to recognize the extraordinary contributions of case managers across the continuum of care.
The outcomes we celebrate in healthcare do not happen by accident. Reduced readmissions. Improved transitions of care. Enhanced patient experiences. Better resource utilization. Stronger interdisciplinary collaboration. These outcomes are achieved because case managers bring clinical expertise, critical thinking, advocacy, and compassion to every interaction.
Your work often happens behind the scenes, but its impact is felt throughout the healthcare journey. Patients, families, providers, and organizations are all stronger because of what you do.
Reignite
The work of case management is rewarding, but it can also be demanding.
That is why this week is an opportunity to reignite our passion for the profession and for the mission we share. It is a chance to celebrate successes, learn from one another, embrace innovation, and renew our commitment to improving lives.
As healthcare continues to evolve, case managers remain uniquely positioned to lead change, solve complex challenges, and champion patient-centered care.
Rise
The future of healthcare requires collaboration, innovation, and leadership. It requires professionals who can bring people together around a common goal and remain focused on delivering better outcomes.
Case managers have always risen to meet those challenges.
From my current perspective in healthcare technology, I see tremendous opportunities to improve connectivity, streamline workflows, and provide better tools to support care coordination. Technology can enhance processes and empower organizations, but it cannot replace the expertise, judgment, and human connection that case managers bring to the profession every day.
That is why case management will continue to be essential in shaping the future of healthcare and driving outcomes that matter most.
Thank You
To every case manager, care coordinator, social worker, discharge planner, utilization management professional, and patient advocate: thank you.
Thank you for reconnecting patients with the care and resources they need.
Thank you for recognizing opportunities to improve outcomes.
Thank you for reigniting hope during difficult moments.
And thank you for rising every day to meet the challenges of our healthcare system with dedication, expertise, and compassion.
Happy National Case Management Week 2026.
