The North Cumbria Integrated Care NHS Foundation Trust has extended its partnership with Strata Health UK, securing a 5-year contract to implement their Transfer of Care Hub and system-wide eReferral platform.
Having been integral to North Cumbria’s healthcare infrastructure for over a decade, Strata Health has successfully adapted numerous changes and mandates in healthcare initiatives. In recent years, amidst organisational restructuring and the division of Cumbria into two separate areas, the need for a digitally supported and mature Transfer of Care (TOC) process became apparent.
Leveraging Strata Health’s specialised expertise in TOC processes, a collaborative project commenced earlier this year. Key project milestones included:
- Developing a Digital TOC electronic single assessment form
- Creating a streamlined end-to-end process
- Enabling seamless care placement
- Integrating with vital systems
- Implementing automated alerts for patients encountering placement delays
- Generating comprehensive data metrics for informed decision-making
- Efficiently managing referrals for care home placements
- Providing real-time updates on bed vacancy status
The innovative solution promises a multitude of benefits, including minimised data entry, improved communication among stakeholders, refined commissioning decision-making, and precise real-time updates on bed availability in care homes.
