Last week, East Lancashire Hospitals Trust (ELHT) implemented Strata Health’s eReferral and Transitions of Care Platform to streamline their ‘Trusted Assessor Approach.’ This NHS-driven initiative aims to decrease delayed discharges by ensuring safe and timely transitions from the hospital to adult social care services.

ELHT’s Trusted Assessor Document (TAD) project involves over 300 users collaborating to create and share essential information and documentation for appropriate service bookings and arrangements to manage ongoing patient care needs post-discharge. These Trusted Assessors play a pivotal role in assessing hospital patients on behalf of HomeFirst, care homes and other providers, evaluating their needs, and coordinating suitable care arrangements.

Strata Health’s digital solution has introduced crucial endpoints such as:

  • HomeFirst
  • D2A Bed base
  • Resi Rehab
  • Complex Pathways
  • Social Care Caseload
  • Community Rehab
  • Integrated Therapy Referral

Each of these endpoints is handled by specialists who assess referrals, make informed decisions about patient needs, and coordinate necessary services from transport to nursing care, facilitating smoother discharges and freeing up crucial bed space.

The platform significantly enhances communication and data flow, reducing the burden on nursing staff and ensuring patients are discharged to the most suitable setting. While streamlining processes, it also maintains essential communication between teams, providing them with mandatory datasets for effective decision-making.

Previously, the Trust encountered challenges with paper-based systems and a brief period using Word documents on SharePoint. The transition to Strata’s platform was eagerly anticipated by users seeking a more efficient and reliable system.

“The new trusted assessment digitally enabled by Strata has now gone live. It has been well received by all the user groups in the hospital and wider system partnership. It is improving the quality of our transfer of care information ensuring that community plans for patients can be developed at pace. We have only received positive comments so far.  This is a brilliant step forward for integrated discharge services and helps us to deliver a personalised care approach”

“We take immense pride in this project, made possible by the Unified Tech Fund,” stated Clint Schick, Chief Executive at Strata Health UK. “This achievement not only marks a successful launch but also opens doors for expanding into additional teams and services, supporting East Lancashire on its path toward a more efficient and effective healthcare system.”
