Optimising Healthcare Referrals for Over 555,000 Residents 

(7 September 2023) After an exciting and transformative development period, filled with dedication and hard work, the Cornwall and Isles of Scilly Integrated Care Board (ICB) launched Strata Health’s platform on 4 July 2023. The system’s launch day surpassed all expectations, generating an impressive volume of referrals.

Acting as a unified electronic referral platform across the entire Integrated Care System (ICS), the solution effectively streamlines patient referrals across various sectors, including Primary, Secondary, Community, Social Services, and the 3rd Sector. Through the consolidation of all patient referrals, the ICS can establish a cohesive and efficient approach to delivering care for a population of over 555,000 residents.

With a robust network of more than 250 referring locations and a fully operational clinical portal, the Strata Health platform enables seamless patient transfers across three Integrated Transfer of Care (ITOC) Hubs and 15 primary clinical pathways.

These pathways cover a wide range of services, including:

Within the first day of operation, the system received 462 referrals, followed by an impressive 472 referrals on the second day.

Currently, there are over 2,500 users, and this figure is projected to increase as more features become available, including more integrated workflows. Phase 1A, scheduled for implementation over the next two months, will concentrate on further strengthening and improving workflows and functionality, incorporating invaluable feedback from users.

Looking ahead, the ICS will have the capability to track and monitor important performance indicators, including the acceptance of ITOC referrals within 15 minutes and the triage process within 30 minutes, along with other critical factors. This will be made possible through the implementation of AI-powered reporting and dashboards, providing valuable insights, and facilitating more effective decision-making for improved operational efficiency.

The project and contract for partners is being managed by the NHS’s Cornwall IT Services (CITS).
