Dear Colleagues and Friends,
It is with great pride and gratitude that I join you in celebrating National Case Management Week 2025. This week is more than an annual recognition—it is a reminder of the extraordinary role case managers play in shaping healthcare, and the profound impact we have on the lives of patients and families every single day.
This week is a time to honor the compassion, skill, and relentless dedication of case managers across the healthcare continuum.
This year’s themes from our leading professional organizations capture the very essence of what case managers do each day:
- Guiding Journeys (ACMA & CCMC) reminds us that case managers are more than coordinators—we are navigators, advocates, and partners who help patients and families find their way through complex care transitions. We are the bridge between uncertainty and clarity, between need and access, between hope and healing.
- Innovate, Advocate, Celebrate (CMSA) reminds us that our profession is as forward-looking as it is compassionate. We innovate by finding new ways to connect people to resources, we advocate for patients and families when their voices may not be heard, and we celebrate not just our achievements, but the courage and resilience of those we serve.
At Strata Health, we are deeply committed to supporting case managers in these missions and we see these principles come to life in every partnership and every patient story. We are privileged to support case managers with technology to simplify and strengthen transitions of care, but it is your dedication, empathy, and creativity that bring these tools to life and transform the patient experience. Every time you solve a challenge, comfort a family, or ensure that a patient doesn’t fall through the cracks, you embody the very best of healthcare.
I am fortunate to have experienced this work from two perspectives: the strategic lens of how systems can improve, and the human lens of what it feels like to walk alongside a patient in their most vulnerable moments. It is this dual perspective that reinforces my belief—case management is not only essential to healthcare, it is the heart of healthcare!
This week, I encourage you to take a moment to reflect on the journeys you have guided, the advocacy you have championed, and the lives you have touched. Celebrate your impact, because it is immeasurable.
