Andrew Irvine, Chief of Community announced, “Today we mobilized a project to be undertaken by Strata Health Ltd. as one of our strategic partners, to work with us on real time resource matching across the system using an algorithm based approach”. He further noted, “ The Rotherham Foundation Trust (TRFT) has been planning the project over an extended period of time with an aim to prove the concept as part of a strategy to scale up across the entire local health and social care economy, provided the prototype is successful”.

The initial scope of the project will include the following areas:

  • Section 2, section 5 processing and co‐ordination towards effective discharge to appropriate services
  • Intermediate Care (fixed locations and home based offerings)
  • District Nursing Teams across the Community
  • Planned services such as Podiatry

In addition, robust reporting functionality and real time business intelligence will be developed and be fit for purpose.

Brian James, Chief Executive TRFT commented, “Future service integration will result in the delivery of a much more cost effective NHS, but this will only be achieved through the creation of more integrated pathways and more seamless transition of patients across that pathway and between different Providers within that pathway. Key to reducing costs whilst maintaining and indeed improving quality will be to ensure that the patient is referred to the most appropriate service to meet their needs.”

He went on to say, “ Addressing this problem will require two fundamental changes to the way the NHS currently operates.

Firstly, the NHS must understand the real needs of patients and how these can be best met, at the least cost. The current range of service options has not fundamentally changed in decades, and yet society and its needs have advanced beyond all recognition in that timescale. Most importantly, there is a need to support a fundamental shift within society that moves from a state of cultural dependency of the citizen on the State, to one of independence, where the citizen is empowered to self-care, and the State is an enabler of self‐care rather than a Provider of care.

Secondly, service options available to patients need to be personalised (matched to need and preference), visible and accessible in real time. The way services are currently organised and delivered does not enable progress towards personalisation and therefore cannot support a cultural shift to greater autonomy by the citizen. The Project aims to address these issues and to make available real time options that are appropriate to patients’ needs, in the event that such a need is identified”.

Outbound analysis will include exploring the contact centre and its ability to expand into a “single point of access” going forward . It is understood that existing systems within the community and our new EPR functionality will be leveraged from as appropriate, to gain extended functionality in patient resource matching, capacity management and placement.
