Strata Health Implements eStroke Referral Management Solution for Toronto Stroke Networks

Strata Health Canada is pleased to announce another successful electronic referral management solution implementation. The Toronto Stroke Networks have chosen Strata Health’s Resource Matching & Referral (RM&R) technology to manage and automate the patient referral process across the stroke care spectrum in the Toronto Area.  

Strata Health’s electronic workflow will optimize flow of stroke patients, matching patients to bed availability. The new solution replaced an existing system and will help health professionals guide their patients to appropriate and available stroke rehabilitation services. In keeping with the previous stroke referral system, the new system will support tracking outcomes of patients receiving those services.
