Expanding interoperability is a hallmark of efficient clinical systems in today’s health systems. However, as this new data sharing and integration drives enormous new data capture; new tools and methodologies must be engaged to make productive use of this new data resource on behalf of patients and clinicians.
Embodying this view, Central London Community Healthcare (CLCH) NHS Trust have now implemented Phase 2 of their Child Health project, with Strata Health’s (SHL) workflow and eReferral platform at its core. This work will be one of several abstracts featured at this year’s King’s Fund Congress.
The project focus was to enable automation and data sharing between Acute, GP and Community services, underpinned by interoperability and a standardized, consistent approach to data sharing from the referring acute hospitals and GP’s surgeries. The project has now entered the UAT phase, providing automated screening, acceptance and management of referrals from Maternity departments, Emergency and Urgent Care centres as well as GP practices, sent in to CLCH, ensuring visibility of the patient during this journey. - Evolution of existing clinical & operational processes
The adoption of Strata Health by CLCH to implement clinical workflow automation that provides required, verified and validated information to the receiving CLCH Hub teams, has ensured the receipt of complete, auditable and appropriate referrals removing unnecessary administrative effort, such as clinicians manually chasing information. Furthermore, this bi-directional workflow automation is data rich, enabling real time operational benchmarks and quantitative best practice to be optimized within the Trust.
