News: Canadian Palliative Care Conference 2007 Strata Health Palliative Software Innovation Featured at National Conference

Calgary, Alberta – August 23rd, 2007: The continued partnership between the Calgary Health Region (CHR) and Strata Health Solutions is getting National attention once again.

The Calgary Health Region and Strata Health Solutions are teaming up to co-present “Strata Resource Matching and E-Referral™: Adapting a Web-based Computer Software Program to Streamline the Central Access and Waitlist Process to Hospice” at the annual Canadian Hospice Palliative Care Conference in Toronto, ON – November 4 – 7, 2007.

Brenda Hale, Director of Client Services comments “This is a wonderful opportunity to demonstrate Strata Resource Matching and E-Referral’ flexibility as our continued partnership with CHR continues to expand opportunities to streamline patient waitlists and enhance patient / family outcomes”.

This presentation will highlight strategies, challenges, and successes in development, implementation and evaluation of Strata Resource Matching and E-Referral™ within the CHR’s Palliative and Hospice Care Service.

The Calgary Health Region’s Palliative and Hospice Care Services are provided through a range of care settings working in collaboration and cooperation to provide a unique and complimentary range of service options. To ensure that these services are being accessed or utilized in an appropriate, cost-effective and equitable manner, the Palliative and Hospice Care Service has worked collaboratively with CHR’s Transition Services to develop a centralized access and waitlist management service delivery model for hospice services utilizing a matrix management model. The waitlist process is now being streamlined using Strata Resource Matching and E-Referral™; a web-based computer software program developed by Strata Health Solutions.
