Surrey, British Columbia: January 1st, 2011: Strata Health today announced that Fraser Health has expanded its agreement for the use of Strata PathWays™. The extended Agreement includes the expansion of the Strata PathWays™ solution to the Assisted Living environment over the next few months.
“In the past four years, we’ve worked with Strata Health to realize the benefits of the Strata PathWays™ solution in improving clinician workflow, optimizing the patient journey and reducing wait times. We’ve partnered to push the boundaries of innovation with the development of the Strata IQ™ Patient Flow Dashboard, which has served to improve our daily decision-making regarding patient placements and strategic decisions. They are not like other vendors. They seek to understand our challenges, help us to create a vision and have partnered with us for the long-term. They are invested in the outcomes. We look forward to continuing the journey with Strata Health,” commented Keith McBain, Director of Business Development and Contracts at Fraser Health.
Fraser Health first implemented the Strata PathWays™ solution in February 2007 to facilitate the matching of patients from community and acute care settings to the most appropriate residential care settings. Plans are underway to further expand the use of the solution to Assisted Living environments by March 2011.
Strata Health is currently deployed across Canada and in the UK, sustaining 100% customer retention since its establishment. “We feel privileged and honored to work with customers such as Fraser Health. Their challenges and vision drive our firm’s innovativeness, ultimately improving the experience of patients within the health care system,” commented Don Schick, CEO of Strata Health.
Strata PathWays™ is available to improve patient flow across the care continuum, from GP practices, to acute care to residential care, assisted living, mental health and addiction services, palliative care/hospice, home care, surgical specialists and many other care destinations.
About Fraser Health:
Fraser Health (FH) provides a wide range of integrated health services – ranging from hospital care, to community-based residential, home health, mental health and public health – to the largest and fastest growing population in BC. Its facilities include hospitals, clinics, community health care centres, residential care and assisted living homes. The health authority is committed to improving the health of the population and the quality of life of more than 1.6 million people living in BC communities from Burnaby to White Rock to Hope.
In the residential care sector, Fraser Health funds approximately 6,000 residential care beds to provide care for seniors with complex care needs. In addition, it owns and operates a further approximately 1, 700 residential care beds. The challenges of managing this sector require accurate and detailed information about the patient (resident) flow across the health authority in order to make key resource allocation decisions supported by real time information. Given the scope and breadth of the sector, leaders in FH require performance and monitoring information to be available in a dynamic, easy-to-access presentation that points out ‘hotspots’ combined with an ability to leverage historical data to enhance our ability to adjust business processes to meet client and system needs.
